At every bi-monthly meeting of these Stakeholder Group, the "Theory of Change Update" highlights strategies and evaluation data for review and recommendations for improvement are made by work groups. Over time, the "Theory of Change Updates" and the summaries of the Stakeholder Group discussions and reports on Key Outcome indicators and measures comprise a record of the considerations made to continuously improve the Impact System of Care.
